Cancellation Request



I am requesting to cancel my membership at The 12. I understand all memberships require a 1 Month WRITTEN NOTICE and if my next billing date falls within 1 Month of submitting this form I will be billed again and I should expect 1 final payment after the submission of this form. I understand that any past due balances are still my responsibility and my cancellation will NOT be processed until any due balances are cleared.

I understand this form is a request to cancel my membership and must meet the criteria listed above. Further, I recognize I am responsible for all fees until my cancellation is considered effective and by signing this form I authorize The 12 to draft all amounts owed by any EFT or Credit Card accounts provided to the Studio.

Membership Freeze Request



I am requesting to put my membership at The 12 on hold. I understand the first 30 days are free (Over a 12-month period starting from the start date of the membership year) and any additional time is $15 per month in place of my regularly scheduled auto-payment. I acknowledge my regular payments will resume once this hold is lifted unless another form is submitted.

Your freeze request will be processed within 5 business days. A Studio Manager will contact you if your request is missing any required criteria or there are any issues. A copy of this form is available upon request. Thank you!
